血清Hcy、IL-17C水平与癫痫患者抑郁状态及认知障碍的关系

摘要

[摘要]目的 探讨血清同型半胱氨酸(Homocysteine,Hcy)、白细胞介素-17C(Interleukin-17C,IL-17C)水平与癫痫患者抑郁状态及认知障碍的关系。
方法 选取2022年10月至2025年8月本院收治的105例癫痫患者为研究对象,采用汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)、蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)分别评估患者抑郁状态及认知功能,根据评估
结果划分为癫痫组、合并抑郁状态组、合并认知障碍组、共病组。采用Spearman相关性分析抑郁状态、认知障碍与血清Hcy、IL-17C水平相关性。
结果 105例癫痫患者筛查单纯癫痫患者56例、合并抑郁状态12例,认知障碍19例;合并抑郁状态及认知障碍18例。合并抑郁状态组、合并认知障碍组、共病组的病程长于癫痫组,血清Hcy、IL-17C水平高于癫痫组,且共病组的病程更长,Hcy、IL-17C水平更高(P<0.05),其他临床资料比较差异无统计学意义(P>0.05)。HAMD评分与血清Hcy、IL-17C水平呈正相关(P<0.05);MoCA评分与血清Hcy、IL-17C水平呈负相关(P<0.05)。
结论 癫痫患者抑郁状态及认知障碍发生率较高,且抑郁状态及认知障碍与血清Hcy、IL-17C水平有关。

ABSTRACT

[Abstract] Objective To explore the relationships between serum homocysteine (Hcy) and interleukin-17C (IL-17C) levels and depression as well as cognitive impairment in patients with epilepsy.
Methods A total of 105 patients with epilepsy admitted to our hospital from October 2022 to August 2025 were enrolled as the research subjects. The Hamilton Depression Scale (HAMD) was used to assess the severity of depression, and the Montreal Cognitive Assessment (MoCA) was applied to evaluate cognitive function. According to the evaluation results, the patients were divided into epilepsy group, depression group, cognitive impairment group and comorbidity group. Spearman correlation analysis was used to analyze the correlation between depression, cognitive impairment and serum Hcy, IL-17C levels.
Results Among the 105 patients with epilepsy, 56 cases were simple epilepsy, 12 cases were combined with depression, and 19 cases were cognitive impairment. Eighteen cases were combined with depression and cognitive impairment. The disease courses of the depression group, cognitive impairment group and comorbidity group were longer than those of the epilepsy group. The levels of Hcy and IL-17C were higher than those of the epilepsy group. Moreover, the disease course of the comorbidity group was longer and the levels of Hcy and IL-17C were higher (P<0.05). No statistically significant differences were observed in other clinical data (P>0.05). HAMD score was positively correlated with serum Hcy and IL-17C levels (P<0.05). MoCA score was negatively correlated with serum Hcy and IL-17C levels (P<0.05).
Conclusion  s The incidence of depression and cognitive impairment is high in patients with epilepsy, and depression and cognitive impairment are related to serum Hcy and IL-17C levels.
